Completed forms should be submitted to your child's Head Coach, together with the applicable membership fee which the Head Coach will have explained to you as part of our initial onboarding process.
Please note: It is club policy that only paid-up members can train with or play matches for the club. Otherwise our public liability and personal accident insurance cover will not apply.
